PATIENT JOURNEY: optimize the patient’s journey

We analyze the patient journey, that is, the journey of the person with a disease along his or her care pathway, while supporting you in understanding and optimizing the diagnostic-therapeutic care pathway (in italian PDTA), putting the patient’s needs at the center.

We design the ideal patient pathway

The Patient Journey is the journey a patient takes from the onset of symptoms, through diagnosis, to the administration of therapy and recovery or care management of a chronic condition. Understanding this journey translates into a patient journey mapping defining the obstacles they face (inefficiencies, unclear pathways, lack of funding, etc.) that are limiting access to therapy.
We can help you design the ideal patient care pathway, which should then be the model for PDTA implementation.

The PDTA, the key tool for an optimal pathway

A PDTA is a national, regional, and/or local document that contains a set of recommendations and guidelines that define the ways, times, and procedures to be followed to care for a patient with a specific condition. A well-designed and adopted PDTA ensures:

  • More effectiveness: Reduced waiting time and improved clinical outcomes.
  • More efficiency: Optimized resources and reduced costs.
  • More quality: Personalized care centered on the patient and his or her needs.
  • More safety: Reduced risk of errors and complications.

Patient Journey and PDTA with Helaglobe

For more than a decade, we have been building strong and lasting relationships with patient associations. We actively listen to their voices, deeply understanding the needs and challenges faced by people with pathology throughout the care journey. Through this experience, we have developed deep expertise in analyzing patient pathways.

Each person is unique and needs a personalized diagnostic-therapeutic care pathway. We help you create pathways that put individual needs at the center.

Home infusion: evidence-based dialogue with regions

We supported the Italian Association “Anderson-Fabry” and the Italian Association “Gaucher” in their dialogue with the Emilia-Romagna region to make sure that people with lysosomal storage diseases could receive infusion of life-saving enzyme replacement therapy at home.

To achieve the result, we created tangible evidence (differential analysis and Algorithm of Choice) on the value of home therapy for a specific type of people through the involvement of clinicians from the Lysosomal Disease Referral Centers and patients in the region, and facilitated the meeting with regional political and technical bodies.

On September 15, 2022, the Emilia Romagna Region, gave the green light to home infusion.
